Key Responsibilities\n- \n

- Provide emotional support and counseling to seniors and their families, guiding them through transitions such as health challenges, relocation, or loss. \n

- Work collaboratively with interdisciplinary teams, including healthcare providers, nurse practitioners, and care coordinators, to ensure a comprehensive care approach. \n
- Regularly monitor clients’ well-being through scheduled check-ins and visits, adjusting care plans as necessary based on observations and feedback. \n
- Facilitate communication and conflict resolution among family members involved in the clients’ care, ensuring a cohesive support network. \n
- Educate clients and their families about available community resources, fostering connections to enhance social engagement and community involvement. \n
- Maintain accurate documentation of client assessments, care plans, and ongoing progress notes in compliance with organizational and regulatory standards. \n
- Stay informed about the latest practices in social work and eldercare to uphold ethical standards and best practices in service delivery. \n
This role primarily focuses on supporting the senior population, which includes individuals facing various life transitions, chronic health conditions, or the challenges that accompany aging. Qualifications\n- \n
- Master’s Degree in Social Work from an accredited institution. \n
- Current and valid licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania. \n
