Job Details:

• Schedule: Days; 8 hours per shift; minimum of 40 hours per week
• Required Qualifications: Valid California Physical Therapist License, at least 1 year of experience as a Home Health Physical Therapist, valid California driver's license, and auto insurance. BLS certification is preferred.
• Certification: BLS (Basic Life Support) preferred
• Skills: Gait training, assessment, patient family education, therapeutic exercises, mobility aids, and safety and injury prevention
• Patient Age Groups: Adults, Geriatrics
• Daily Census: 80 patients
• Number of Visits Per Day: 5 visits
• Unit Details: Interdisciplinary support available including Physical Therapy, Interpretation Services, and Social Services
• Additional Information: Position begins on 08/03/2026, with orientation hours totaling 40. All documentation must be submitted and verified 10 days prior to start. Candidates must provide evidence of recent licensure via Nursys and applicable certifications with valid expiration dates.
