Nurse Practitioner/Physician Assistant

We are seeking a Nurse Practitioner or Physician Assistant to join an established adult primary care practice within a comprehensive healthcare setting. Responsibilities include providing primary care for adult patients , conducting wellness visits, managing acute and chronic conditions such as hypertension, diabetes, and COPD, and developing personalized treatment plans focused on prevention and disease management. Key duties involve ordering and interpreting diagnostic tests, educating patients on medications and lifestyle changes, collaborating with a multidisciplinary team, utilizing telemedicine, and maintaining accurate documentation in the electronic health record. Qualifications include current licensure in New York , graduation from an accredited NP or PA program, DEA registration or eligibility, and strong clinical decision‑making skills. Preferred experience in Internal Medicine, Primary Care, or Urgent Care , with a focus on managing chronic diseases.
