Position Summary: Home Health Aid/ CNA Job Description A Home Health Aide is a non-licensed member of the home care team who assists the client with the tasks of daily living as outlined in a written care plan that is established by a the Administrator or CSS and is kept in the clients home. Home Health Aides are supervised by the Administrator agency and receive scheduling direction from the agency staffing Coordinator. Qualifications: 1. A Home Health Aide must have a minimum of one year of personal care experience which is verifiable through three work references and must complete and pass this agency’s employment and competency process. 2. Must be at least 18 years of age. 3. A Home Health Aide must have training as a Home Health Aide and complete a competency evaluation. 4. Must be physically able to perform the duties of the position, including lifting unspecified weights, depending on assignment requirements. Bending and stooping to prepare equipment for client or managing client environment. Transferring, turning and positioning clients and assisting to ambulate. 5. Must exhibit mature responsible behavior and understand the need for patient confidentiality. 6. Must be able to read, write and speak English. 7. Must be able to follow direction and accurately report to the Administrator any changes in clients condition. 8. Must have available reliable transportation to and from assignments. May be approved to transport client or run errands in vehicle. 9. Certified Nursing Aide (preferred) Essential Functions: 1. The HHA shall be assigned to a client by an RN. Written instructions for client care shall be prepared and updated as needed by an RN. 2. The HHA may provide services that are ordered by the physician in the plan of care and that the HHA is permitted to perform under state law and that are consistent with the HHAs training. 3. HHA services provided to 4 Your Peace of Mind Home Health clients may include, but not be limited to, the following duties: a. The provision of hands on personal care including, but not limited to assisting clients with personal grooming and hygiene such as skin, mouth, hair care, shaving, dressing and bathing. b. Performance of simple procedures as an extension of therapy or nursing services; c. Assisting clients in and out of bed, assisting with ambulation or exercises. d. Nutrition/hydration and assisting clients with eating. e. Assisting clients with toileting and elimination. f. Performing household services essential to health care at home which will facilitate the clients self-care at home and are necessary to prevent or postpone institutionalization. g. Assisting clients with medications which are ordinarily self-administered. Refer to 4 Your Peace of Mind Home Health Administration of Medication Policy for requirements for the provision of medication reminding services for clients who can self-direct. Supervision of self-administered medication in the home is limited to the following: ● Obtaining the medication container from the storage area for the client. ● Ensuring that the medication is prescribed for the client. ● Reminding the client that it is time to take the medication as prescribed, and; ● Observing the client self-administering the medication. a. Performing other activities delegated and taught by an RN for a specific client in accordance with Connecticut Statutes and Administrative Codes and 4 Your Peace of Mind Home Health Nursing Service Policies. These may include such services as: ● Assisting with changing colostomy bags/reinforcement of dressing. ● Assisting with the use of devices for aid to daily living, such as a wheelchair or walker. ● Assisting with prescribed range of motion exercises. ● Assisting with prescribed ice cap or collar. ● Doing simple urine tests for sugar, acetone, or albumin. ● Measuring and preparing special diets. ● Measuring intake and output of fluids. ● Measuring temperature, pulse, respiration or blood pressure. b. Observing appearance and gross behavioral changes in the client and reporting changes in the client;s condition and needs to the RN case manager or Home Health Clinical Manager. c. Completing records regarding services performed. Documentation is submitted by end of business day. d. The HHA may NOT provide the following services: ● Change sterile dressings. ● Irrigate body cavities (i.e. giving an enema, irrigate a colostomy or wound, perform a gastric irrigation or enteral feeding). ● Catheterize a client. ● Administer medication. ● Apply heat by any method. ● Care for a tracheostomy tube. ● Provide any person health service which has not been included in the plan of care. 4. In addition to the requirements outlined in the 4 Your Peace of Mind Home Health Orientation Policy, the HHA and HM shall be oriented and demonstrate competency in any service the employee or contractor is to perform prior to providing client care. Competency assessments shall be documented by the Home Health Clinical Manager or another RN who possesses a minimum of two (2) years of “hands on” clinical nursing experience (direct provision or care and/or supervising nursing services or teaching nursing skills in an organized curriculum or in-service program), at least one (1) year of which must be in the provision of home health care. Subject areas must include, but not be limited to the following: a. Ethics, including confidentiality of information. b. Client Rights and Responsibilities. c. Communication skills, including the ability to read, write and verbally report clinical information to clients, representatives, and caregivers, as well as to other Agency staff. d. The physical, emotional, and developmental needs of and ways to work with the populations served by 4 Your Peace of Mind Home Health, including the need for respect for the patient, his or her privacy, and his or her property. e. Consumer control and the independent living philosophy. f. Instrumental activities of daily living. g. Observation, reporting and documentation of client status and the care or service furnished. h. Recognizing changes in the consumer that need to be addressed. i. Handwashing and 4 Your Peace of Mind Home Health Infection Prevention and Control policies and procedures. j. Universal precautions. k. Body mechanics. l. Maintenance of a clean, healthy and safe environment. m. Fire safety. n. Recognizing emergencies and knowledge of instituting emergency procedures and their application. This includes knowledge of 4 Your Peace of Mind Home Health policies for handling of medical and non-medical emergencies. o. Assignment and supervision of services. p. Reporting complaints or grievances. q. Recognizing and reporting abuse or neglect. r. Dealing with difficult behaviors. s. Each employee’s specific duties and responsibilities. t. The roles of, and coordination with other community service providers. 5. An orientation and competency program developed by 4 Your Peace of Mind Home Health for an HHA who will provide personal care must address the following additional subject areas: a. Bathing (sponge, tub, or shower bath), shaving, grooming and dressing. b. Hair care (including shampoo in sink, tub or bed), skin, nail and mouth care. c. Assistance with safe ambulation and transfer techniques. d. Adequate nutrition, hydration, meal preparation and feeding. e. Toileting and elimination. f. Reading and recording temperature, pulse and respiration. g. Assistance with medication reminders for medications that are normally self- administered. This includes but is not limited to what constitutes medication reminding and its specific limitations. h. Appropriate and safe techniques in personal care tasks. i. Normal range of motion and positioning. j. Methods and techniques to prevent skin breakdown, contractures and falls. k. Basic elements of body functioning and changes in body function that must be reported to the Support Services Supervisor or Home Health Clinical Manager. l. Any additional training identified by 4 Your Peace of Mind Home Health which is necessary to meet the needs of the population served by the agency.
