Social Worker (LSW/CSW) Qualifications & Responsibilities: Must possess a current New Jersey LSW or CSW license. Previous subacute and/or long-term care experience preferred. Complete resident Social Histories and Psychosocial Assessments. Help identify residents' psychosocial, emotional, and mental health needs. Collaborate with the interdisciplinary team to promote and protect resident rights. Prevent, recognize, and address resident abuse in accordance with applicable laws and professional standards. Maintain accurate documentation and resident records. Experience with PointClickCare electronic medical records preferred. Ability to build and maintain positive relationships with residents, families, and team members. Ability to work independently and as part of a team.
