Help patients remain comfortable, safe, and independent. Observe changes in condition and communicate with the hospice team. Provide encouragement, companionship, and emotional support. Follow individualized care plans with professionalism and compassion. Work closely with nurses and the interdisciplinary team to provide exceptional care. Who We're Looking For Hospice experience preferred, with home care, assisted living, long-term care, hospital, or oncology experience also welcomed. Compassionate, dependable, and relationship-focused. Strong communication and teamwork skills. Valid driver's license and reliable transportation.
