Hours:Monday - Friday | One Half-Day Saturday per Month | 10 Hour Shifts 4 Days per Week or 8 Hour Shifts 5 Days per Week

Call Schedule: Phone Only | Rotated Weekly Every 7 Weeks

Sign-On Bonus: $10000
Compensation: - Based on experience- Quarterly incentives Bonus Program for all Physicians-Sign-on bonus!- Relocation assistance provided for FTE!
Benefits: - Medical/Dental/Vision- Malpractice/Tail Coverage- $2K CME Allowance with 5 Days Paid Time Off- 3 Weeks Paid Vacation. Additional Info: Position will spend two days at Chicago Heights location, and two days at South Holland, and one day telemed.- 10 Hour Shifts 4 Days per Week or 8 Hour Shifts 5 Days per Week- Provide continuous, comprehensive primary care for infants, children, youth, and adults according to current standards of care- Make diagnoses and prescribe medication as necessaryQUALIFICATIONS:- IL License REQUIRED
