LPN MedSurg WMLM

Role: Licensed Practical Nurse (LPN) providing direct patient care within a home health setting. Responsibilities Assess and manage patient needs following physician orders with RN oversight, performing routine nursing procedures. Coordinate care with RNs and team members, monitor patient condition changes, and update care plans accordingly. Serve as a patient educator to promote self-care and caregiver support, ensuring safety and effective communication. Complete accurate documentation within required timeframes, participate in team conferences, and adhere to regulatory guidelines. Assist in staff education, participate in ongoing training, and serve as a preceptor for new LPN employees. Qualifications & Skills High School Diploma or GED; valid LPN license and BLS certification. Minimum one year of medical/surgical nursing experience preferred. Strong assessment, clinical, organizational, and communication skills. Proficiency with computers, home health software, and electronic communication tools.
