Role Description The Case Manager is a full-time, on-site role based in the Los Angeles Metropolitan Area. This position is responsible for managing a caseload of individuals and families, conducting intake assessments, and developing person-centered service plans that promote independent living and wellness. Daily responsibilities include coordinating resources and referrals, providing mentorship and guidance, monitoring progress toward goals, and documenting all services in accordance with organizational standards. The Case Manager collaborates with community partners, service providers, and internal team members to ensure comprehensive support for each participant. The role also involves advocating for client needs, facilitating life skills and wellness activities, and participating in regular team meetings and trainings. Qualifications Experience in case management, social services, or related community support roles, preferably working with individuals and families experiencing poverty or housing instability. Strong skills in assessment, service planning, and resource coordination, with the ability to manage a caseload and track progress effectively. Effective communication and interpersonal skills, including the ability to build trust, provide mentorship, and work with people from diverse backgrounds in a trauma-informed and culturally responsive manner. Organizational and documentation skills, including maintaining accurate records, using case management or data systems, and following program and compliance requirements. Ability to work on-site in the Los Angeles Metropolitan Area, collaborate in a team environment, and maintain professional boundaries while demonstrating empathy and respect. Knowledge of local community resources, public benefits, and support services related to housing, employment, health, and wellness is highly beneficial. Relevant education such as an associate or bachelor’s degree in social work, psychology, human services, or a related field, or equivalent experience in community-based work.
