Registered Nurse - Case Management - Hospital

Job Description

Job Summary:
The Case Manager is responsible for coordinating and managing patient care in an acute care hospital setting, ensuring efficient utilization of resources and patient outcomes. This role involves collaborating with healthcare teams to facilitate seamless discharge planning and patient support.

Responsibilities:

• Assess patients' clinical needs to develop individualized care plans.
• Conduct utilization reviews to ensure appropriate resource utilization and manage denials.
• Collaborate with medical staff, patients, and families to coordinate care and discharge planning.
• Maintain documentation of patient interactions, assessments, and care plans accurately.
• Ensure compliance with hospital policies, regulations, and certification requirements.
• Utilize clinical software to document and track patient progress and administrative tasks.

Qualifications:

• Possess a Bachelor's Degree in Nursing or related healthcare field.
• Have at least 3 years of case management experience in an acute care environment.
• Be proficient in utilization review processes and denials management.
• Hold a valid state RN license in hand.
• Be BLS certified; additional case management certifications preferred.
• Demonstrate effective communication and organizational skills.
