LPN – Home HealthSchedule: 8hrsAssignment Details: Guaranteed Hours: 40 Contract Weeks: 12Requirements:Current Nursing License in the practicing stateValid Driver's LicenseCurrent CPR CertificationTwo years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice settingGraduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational NursingAbility to use electronic medical charting and time-keeping softwareProof of current Hepatitis consent/declination and TB vaccination recordReliable transportation and current auto liability insurance coverageMust read, write, and speak fluent EnglishGood and regular attendanceResponsibilities:Administer skilled nursing care to patients in their place of residence.Assist in identifying the patient's physical, psycho-social, and environmental needs.Participate in planning, coordinating, and implementing care with an interdisciplinary team.Provide effective communication to patient/family and colleagues.Assist the patient/family in learning appropriate self-care techniques.Utilize infection control measures in providing care.Assist with end-of-life care for hospice patients.Support the admitting, discharging, and transferring of patients.Participate in annual in-service and performance improvement programs.
