Position: Registered Nurse - Home Health

We are seeking a dedicated and professional Registered Nurse to provide home nursing care in accordance with physician orders and agency policies. The ideal candidate will have experience in home health or hospice settings and will work directly under the supervision of the Clinical Manager.

Key Responsibilities:

Coordinate and direct the provision of nursing care in patients' homes.
Conduct patient assessments and apply nursing processes competently.
Supervise and evaluate the performance of Home Health Aides and Licensed Practical Nurses.
Document patient care activities accurately and in a timely manner.

Job Details:

Location: Clinton, SC
Shift: Day 5x8-Hour (08:00 AM - 05:00 PM)
Contract Duration: 13 weeks
Average Number of Patients per Day: 5-10
Travel Requirement: Must be willing to travel up to 75 miles from the home clinic.
Weekly Productivity Expectations: 30 points required to meet productivity goals.

Qualifications:

Minimum of 1 year of experience in Adult Home Health or Hospice care.
Valid nursing license.
Current CPR and BLS certifications required.
Proficient in OASIS and HomeCare HomeBase EMR systems within the last 6 months.

Additional Information:

On-call responsibilities may include 1-2 days during the week and one weekend per month.
Candidates must have reliable transportation, a valid driver's license, and agency-required liability insurance.
Documentation of patient care must be completed within 24 hours of each appointment.
This position may require longer travel times between patients; candidates should be comfortable with this.
