Develop, implement, and evaluate individualized plans of care. Provide skilled nursing care, education, and disease management. Monitor patient progress and identify changes in condition. Collaborate with physicians, therapists, caregivers, and interdisciplinary team members to coordinate care. Educate patients and families on medications, treatments, and disease management. Accurately document visits and maintain timely clinical records. Ensure compliance with all clinical standards, agency policies, and HIPAA regulations. Advocate for patients while promoting the highest quality of care. Qualifications Current, unrestricted Massachusetts Registered Nurse (RN) license. Minimum of one year of nursing experience in a clinical setting. Home Health experience preferred, but not required. Experience with OASIS documentation is preferred. Experience with Homecare Homebase is a plus. Strong assessment, communication, and organizational skills. Associate’s Degree in Nursing required; Bachelor’s Degree preferred. Valid driver's license, reliable transportation, and current auto insurance. Ability to travel throughout the Newton service area. Ability to safely lift, push, pull, and assist with up to 50 pounds as needed.
