Key Responsibilities\n

  • Conduct thorough psychosocial assessments to understand the unique needs and circumstances of each patient and their family.

  • Develop individualized treatment plans incorporating evidence-based interventions aimed at alleviating emotional distress and improving quality of life.

  • Provide therapy sessions using solution-focused techniques and other therapeutic modalities consistent with patient needs.

  • Maintain concise and accurate clinical documentation in compliance with HIPAA regulations and organizational standards.
  • \n
  • Engage with a multidisciplinary team, including physicians and nurses, to deliver comprehensive care that addresses both physical and emotional aspects of patient well-being.

  • Educate patients and families about available resources, coping strategies, and care options to ensure informed and empowered decision-making.

  • Stay current with clinical practices, mental health trends, and trauma-informed approaches to continue to enhance professional knowledge and practice efficacy.

Patient Population / Client Focus\n

In this role, you will primarily work with patients receiving hospice care, many of whom are dealing with terminal illnesses like cancer, heart disease, or neurodegenerative disorders. You will provide emotional support tailored to the needs of these patients and their families, as they navigate complex feelings of grief, anxiety, and sadness associated with end-of-life experiences. Your role is pivotal in fostering a compassionate space where patients can express their feelings and where families can receive guidance and support.

Qualifications\n

  • A Master’s degree in Social Work from an accredited program.
  • \n
  • A valid LCSW license in the state of Georgia.
  • \n
  • A minimum of 2 years of clinical social work experience, preferably in a healthcare or hospice setting.

  • Strong skills in communication and interpersonal interactions, essential for building trust and rapport with clients.

  • Proficient in utilizing solution-focused therapy techniques and other evidence-based therapeutic interventions.

Preferred Experience\n
