POSITION SUMMARY/JOB PURPOSE:
The IDD Service Coordination Team Leader directs and trains staff responsible for IDD Service Coordination Habilitative Coordination and Collaborative Care Case Management (CCCM) services. This position ensures objective monitoring and evaluation of service delivery and provider performance as well as compliance with regulatory guidelines for service coordination, habiltiative coordination, and CCCM services. The Team Leader also serves as the PASRR Diversion Coordinator and ECC Coordinator. This position supervises IDD Service Coordinators.
This position works independently, under limited supervision, reporting major activities through periodic meetings.
EDUCATION, EXPERIENCE, OTHER QUALIFICATIONS:
Education Required: A Bachelor's degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education or early childhood intervention or a bachelor's degree with at least 30 hours of coursework in the previous fields.
Experience Required: At least 2 years experience with individuals with intellectual and developmental disabilities or mental illness preferred.
Registration, Certification, Licensure or other Qualifications Required:
- Must maintain a valid Texas Driver's license, auto liability insurance and a driving record acceptable to PermiaCare's insurance requirements.
- Required to pass criminal history and background checks as well as pre-employment drug screen.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Ensure training and provision of Service Coordination using a person centered approach focusing on the needs and desires of persons served.
- Review and supervise staff performance.
- Support continuous improvement initiatives to improve Service Coordination, habilitative coordination and CCCM.
- Oversee continuation, coordination and delivery of person directed plans to ensure continued services.
- Collaborate with other LIDDAs and private providers to ensure continuity of services.
- Perform tasks of Diversion Coordinator as defined in the HHSC Performance Contract.
- Assess persons for IDD or IDD and MI in nursing facilities.
- Conduct duties for Enhanced Community Coordination as defined in the HHSC Performance Contract.
- Identify available community living options, services, and supports to assist individuals to successfully live in the community.
- Provide information and assistance to IDD Service Coordinators and other LIDDA staff who facilitate diversion for individuals at risk of admission to a nursing facility.
- Coordinate the provision of education to IDD Service Coordinators and other LIDDA staff about available community services and about strategies to avoid nursing facility placement.
- Within 45-75 days of an individual's admission to a nursing facility, review the admission to ensure community living options, services and supports that could provide an alternative to a nursing facility placement have been explored and if not, refer the individual to their IDD Service Coordinator for that purpose.
- Report to the State the number of individuals in the Target Nursing Facility population defined as a Medicaid recipient who is 21 years of age or older with IDD, admitted to nursing facilities, diverted from nursing facilities prior to admission, and residing in a nursing facility for more than 90 days, and provide information to the State about barriers experienced in moving individuals from a nursing facility to the community as directed by supervisor.
- Provide administrative and supervisory duties to ensure effective operation of IDD Service Coordination.
- Perform tasks of Outpatient Biopsychosocial Assessment Intervention as defined in the State Performance Contract.
- Meet unit performance measures or targets.
- Maintain assigned caseload.
- Coordinate appropriate services to designated caseload.
- Enter accurate and appropriate documentation of services within timeframe required.
- Maintain confidentiality of sensitive records and treatment information, client files and protected health information in compliance with HIPAA, laws, rules and regulations, and established procedures.
- Maintain regular and reliable physical on-site attendance. Regular attendance, dependability, and promptness are required for the scheduled work day 100% of the time, to ensure consistency and completeness of program's processes.
- Comply with the Abuse, Neglect, and Exploitation policy and reporting requirements.
- Adhere to the Code of Conduct and Standards of Behavior policy requirements.
- Establish and maintain effective work relationships with individuals served and their families, supervisors, co-workers and visitors by demonstrating cooperative, courteous and respectful behavior at all times.
- Communicate regularly with supervisor.
- Open and process mail/email in a timely manner.
- Answer phone, collect phone messages and respond to requests timely and accurately.
- Maintain safe and clean working environment by complying with procedures, rules and regulations.
- Perform all work functions and interactions using a trauma informed approach.
- Display professionalism when representing PermiaCare and the program in the community.
- Maintain compliance with legal requirements and company policies and procedures.
- Maintain valid and current driver's license, auto insurance, acceptable driving record and reliable transportation at all times. Driving may be required for this position.
- Complete all training as assigned prior to due date.
- Plan, organize, assign, supervise, review, and evaluate the work of assigned employees.
- Participate in the hiring process and termination process of assigned employees, including recommending selection of staff to hire.
- Complete employee corrective actions as needed and in a timely manner.
- Communicate regularly with assigned employees.
- Ensure compliance for performance evaluations for assigned employees.
- Perform approval of timesheets on time.
- Develop, implement, and monitor new and revised operational policies and work procedures.
- Monitor caseloads of assigned staff and assigns work accordingly.
- Coordinate with Human Resources to respond to employee concerns or complaints.
- Establish expectations and provide employee performance feedback on an on-going basis, including ensuring understanding of performance targets and goals.
- Provide orientation and on-going training, mentoring, and coaching to assigned employees.
- Maintain quality and quantity of work performed within work unit.
- Fill in for subordinates when needed to ensure coverage.
- Assist in resolving emergencies.
- Identify and resolve workplace problems.
- Ensure direct reports are aware of adhere to PC policies and procedures.
- Ensure time is reported accurately.
- Other duties as assigned.
