Physical Therapist - Home Health

Location: Oahu, Hawaii

Assignment Duration: This is a 13-week contract position.

Schedule: Days, 8 hours per day

Job Description: We are seeking a dedicated Physical Therapist to join our home health team. The Physical Therapist will plan and conduct segments of treatment programs aimed at restoring and improving physical, social, and mental functions to meet department objectives. The role involves serving as a co-leader in an interdisciplinary approach to patient groups.

Responsibilities

• Complete evaluations to ensure quality standards and address assessment areas requiring attention.
• Utilize appropriate clinical techniques and tools to establish individualized treatment plans.
• Provide therapy interventions using recognized treatment protocols and techniques.
• Develop individualized care and discharge plans while ensuring effective communication with all relevant parties.
• Maintain comprehensive and accurate clinical documentation within established deadlines.
• Supervise assigned assistants in delivering patient care.
• Perform additional job-related duties as assigned.

Required Experience / Certifications / Licensure

• Minimum of 2 years of Physical Therapy experience.
• Current Hawaii state Physical Therapist license.
• Bachelor's degree in Physical Therapy is required; Master's degree is preferred.
• 1 year of home health experience is necessary.
• Experience with Homecare Home Base and OASIS is required.
• CPR and BLS certifications from AHA are mandatory.
• Valid driver's license and current auto insurance.
