Position Summary We are seeking a Clinical Care Coordinator to support our Chronic Care Management (CCM) and population health programs. This role focuses on patient engagement, care coordination, and improving health outcomes through structured follow-up and documentation. Key Responsibilities Coordinate Chronic Care Management (CCM) services Develop and maintain patient care plans Provide patient education and chronic disease support Track care gaps and support quality improvement initiatives Collaborate with providers on care coordination Maintain accurate Medicare and CMS-compliant documentation Support population health reporting and workflows Qualifications Master’s degree in Public Health, Nursing, Health Administration, or related field Clinical background strongly preferred (RN, MD, MBBS, etc.) CCM or care coordination experience preferred Strong knowledge of Medicare and CMS guidelines Medical, dental, and vision insurance Paid time off (PTO) Competitive salary based on experience Hillside Medical Group welcomes qualified international healthcare professionals and offers H-1B sponsorship for eligible candidates.
