Responsibilities:

• Deliver high-quality patient care by assessing patient needs, developing individualized care plans, and implementing interventions to achieve optimal outcomes

• Conduct thorough patient assessments , document patient information, and maintain accurate records
• Communicate effectively with patients, families, and healthcare professionals to ensure smooth patient transfers and care transitions
