Carillon Oaks at Lincoln is seeking a compassionate, dependable Licensed Practical Nurse (LPN) to join our assisted living team.

As an LPN at Carillon Oaks at Lincoln, you will play an important role in supporting the health, safety, and well-being of our residents.

Key responsibilities include:

• Provide compassionate nursing care in accordance with residents' individualized care plans

• Administer medications and treatments as ordered by physicians and in accordance with applicable regulations

• Monitor residents for changes in condition and promptly communicate concerns to the appropriate clinical leadership

• Perform routine nursing assessments and document resident care accurately and timely

• Coordinate with physicians, pharmacies, families, and other healthcare providers as needed

• Respond appropriately to resident needs and changes in condition

• Provide guidance and support to resident care staff

• Assist with admissions, assessments, and ongoing resident care planning

• Maintain accurate clinical documentation and medication records

• Promote resident dignity, independence, choice, and quality of life

• Communicate professionally and compassionately with residents and their families

• Collaborate with leadership and fellow team members to ensure continuity and quality of care

• Follow all applicable state regulations, company policies, and infection-control practices

• Help maintain a positive, respectful, and team-oriented environment throughout the community

• Current and unrestricted Licensed Practical Nurse (LPN) license

• Previous assisted living, senior living, skilled nursing, or long-term care experience preferred

• Strong clinical judgment and attention to detail

• Excellent communication and interpersonal skills

• Ability to effectively prioritize resident needs in a fast-paced environment

• Dependable, organized, and committed to providing high-quality care

• Strong teamwork and problem-solving skills

• A genuine passion for working with seniors and supporting their quality of life
