How you'll make an impact in this role

• Coordinate the interdisciplinary plan of care across the continuum from admission to discharge
• Integrate patient and family preferences while monitoring care through assessments and records
• Advocate for resources, remove barriers, and maintain ongoing dialog with supervisors and providers
• Act as a resource for documentation accuracy, service appropriateness, and post-discharge options

What minimum requirements you'll need

Licensure / Certification / Registration:

• One or more of the following required:
  • Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate'
  degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

What additional preferences we're seeking

Previous hospital experience highly desired
