Must-Have: Experience in skilled nursing facility or long-term care preferred

Description: Schedule: 12-hour shifts from 6:00 AM to 6:30 PM. Resource may work 36 hours the first week and 40 hours the following week. 40 hours per week is not guaranteed. Administer medication and perform treatments as ordered by the physician and within the scope of license. Receive and deliver a variety of information regarding residents' medical, social, and functional needs. Participate in receiving and giving shift reports. Communicate with residents, family, and staff to ensure appropriate care. Perform initial admission nursing assessment on newly admitted residents. Assist with the development of Minimum Data Set (MDS) assessments and ongoing care plans. Maintain clinical and administrative documentation regarding diagnosis, assessments, and treatment. Lead and supervise subordinate staff, assisting with direct resident care as needed. Conduct and participate in in-service training and staff development programs. Utilize problem-solving and continuous improvement tools for efficiency. Knowledge of gerontological/geriatric nursing theory and practices. Understand the effects of the aging process on residents' needs. Familiarity with team nursing practices and treatment plans. Knowledge of pharmacology and medication administration practices. Adhere to professional nursing methods and techniques. Observe and assess behavior and progress of patients. Develop and carry out patient care plans within a team context. Build effective interpersonal relationships. Ability to learn and implement continuous improvement methods.
