Anchor Health is seeking an After-Hours Hospice LVN/RN to provide skilled, on-call nursing care outside of standard business hours. This role ensures continuity of care, symptom management, and emotional support for patients and families after 5:00 PM. The nurse works independently while maintaining close communication with the interdisciplinary team, physicians, and leadership to support individualized plans of care.Schedule5pm-8am, weekends and holidays as assigned, on-call/after-hours coverage.Key ResponsibilitiesRespond to after-hours patient calls and urgent care needsPerform skilled nursing assessments and symptom managementProvide emotional support and education to patients and familiesImplement and update patient plans of care as neededCommunicate patient condition changes to the interdisciplinary teamCollaborate with physicians and caregivers regarding patient needsComplete accurate and timely clinical documentationUphold hospice philosophy, policies, and regulatory standardsQualifications & RequirementsCurrent RN or LVN license in good standingHospice or home health experience preferredStrong clinical judgment and ability to work independentlyExcellent communication and problem-solving skillsCompassionate presence in emotionally sensitive situationsValid driver’s licenseReliable, insured vehicle in good working condition (per state and organizational requirements)What We OfferCompetitive compensationComprehensive benefits packageSupportive leadership and team environmentMeaningful, patient-centered workA culture that values and respects nurses
