Licensed Practical Nurse - LTC

LPNWe are seeking qualified Licensed Practical Nurses (LPNs) to join our team in a long-term care/specialty nursing facility. This position requires a commitment to delivering high-quality patient care and supporting a diverse range of diagnoses.ResponsibilitiesProvide nursing care to patients according to established standards and practices.Assist in the administration of medications and treatments as directed.Monitor patient conditions and report changes to the healthcare team.Maintain accurate patient records in the electronic medical record system.Collaborate with interdisciplinary teams to develop and implement patient care plans.Shift InformationShift Length: 12 hoursSchedule: Three shifts per week, including every other weekend and holidayShift Timing: 0600-1830QualificationsMinimum of 1 year of experience as a Licensed Practical Nurse.Current BLS Certification required.Experience with electronic medical records (EMR), preferably PCC.IV Certification for LPNs is preferred.Working EnvironmentThis position is located in a facility with 128 beds, where the nurse-to-patient ratio is 1:32.
