Clinical Pharmacist

Intermountain Health seeks a Clinical Pharmacist for our outpatient setting on a PRN basis. You will provide direct patient care through comprehensive medication reviews, MTM services, and chronic disease management, ensuring safe, evidence-based therapy. Responsibilities include verifying and dispensing prescriptions, optimizing medication regimens, educating patients and providers, and collaborating with a multidisciplinary team.
