We are teachers, researchers, innovators, leaders and, most of all, caregivers.Some highlights of this opportunity include:Participate in the decision-making process in relation to admission to hospice at home or in the inpatient settingParticipating as a member of the Interdisciplinary Team in the development of individualized care plansCertify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice BenefitConsult with attending physicians regarding pain and symptom managementCoordinate efforts with the attending physician to provide continuity of careDevelop and monitor the medical component of the hospice programParticipate in the development, coordination and implementation of hospice services and policiesParticipating in the development of staff and volunteer trainingConduct home visits (in person and virtual) as a consulting physician when appropriateAct as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and communityPerform related administrative functions as required and/or assignedSchedule: Forty-eight (48) on call weeks per year: Monday 7am through Friday 7pm. Including mandatory in person attendance of 1 Inter Disciplinary Team Meeting (IDT) per week.
