JOB PURPOSE: The Parent Engagement Meeting Facilitator is responsible for Parent Engagement Meetings at designated elementary schools to define barriers to attendance, identify strengths and needs, develop a plan of action, and strive to prevent the family from being unnecessarily involved in the child welfare system.
RESPONSIBILITIES AND DUTIES:- Program planning meetings with any new school systems and/or school staff as appropriate.
- Establish shared Google Calendar for each participating school system.
- Amend PEM forms for each school system.
- Communicate with school staff regarding PEM referrals and scheduling.
- Facilitate the Parent Engagement Meetings.
- Develop action plan for the family.
- Assist with making appropriate referrals to meet identified needs.
- Provide follow-up call to the parent/caregiver within 3 to 4 weeks after the PEM.
- Provide additional follow-up calls as needed or requested.
- Enter PEM information for each meeting into the GenTrack reporting system.
- Attend required state PEM meetings and CCC trainings as scheduled.
- Collaborate with school systems to obtain year end attendance data.
- Attend the Regional Network Meetings and advise community partners about the trends and overview of services.
- Advocate for the best interest of the child.
- Conduct confirmation calls with family prior to meeting to discuss purpose and introduce before meeting – this helps decrease number of “no shows” to meetings
- Works with families for up to approximate twelve-week time period. Staff provides flexible services to families by offering more frequent visits or extending the length of services when needed to meet the family’s needs.
- Carries a 4-5 case load during the summer, ensuring that cases are opened and closed in a timely manner.

- Meets with families for no fewer than 2 hours per week. Evenings or Saturday mornings may be required.
- Assess the strength of the family as well as the issues requiring attention using the referral, North Carolina Family Assessment Scale (NCFAS), and Ages and Stages Questionnaire (ASQ-3). All assessments must be completed within the time frames found in the Regional Guidelines.
- Schedule a home visit within five (5) working days of receiving a new referral. Conduct home visit within seven (7) working days of an accepted referral.
- document observations, goals and interactions during home visits. Information gathered is maintained in the case file as well as the CCC database.
- CKCAC adheres to a policy of confidentiality. A confidentiality form is provided to each family and the In-Home worker signs the form. The form is maintained in the case file. The case file is maintained in a secure, locked drawer in the office to further increase confidentiality.
- Develop written goals with the family based on the results from the NCFAS and collaboration with the family.
- Direct the family in locating community resources.
- Maintain a professional relationship with parents, family members, and child(ren).
- Complete the Potential Safety Plan within the first 24 hours of a home visit. The plan can be updated throughout the intervention if safety becomes an issue. The plan documents the safety measures to put in place.
- CKCAC maintains a log in the family’s case files and maintains progress notes to document contact with the family. A Family Visit Plan that is signed by a family member is maintained in the case record.
- Collect client satisfaction surveys from each family and maintain them in the case file.
- Develop an Aftercare Plan with each family.
- Attempt and document follow-up contact with the family at 6 months to check on the family’s wellbeing.
- Other duties as assigned.
- Ability to lift from the floor to 18” above the shoulder.
- Vision sufficiently correctable to permit full performance of job duties.
- Ability to drive for travel within the service area and beyond, if necessary.
- Wide range of mobility.
- Able to perform clerical functions such as filing, labeling, stamping, stapling, and writing.
- Must be able to lift/manipulate/move objects up to 50 lbs.
- Must hold a bachelor’s degree in Family or Human Services or a related field OR hold a bachelor’s degree in an area outside of human services but have at least one year of experience in providing in-home services.
- Must possess and maintain a valid driver’s license.
- Drug screen and background check required.
