Job Summary:
The Licensed Vocational Nurse (LVN) provides skilled nursing care within a skilled nursing facility under the supervision of a Registered Nurse and Director of Nursing. The LVN is responsible for assessing residents, administering medications with strict adherence to safety protocols, supervising nursing assistants, and collaborating with the interdisciplinary team to ensure continuity and quality of care. The role demands strong clinical skills, effective communication, and a compassionate approach to resident advocacy and education. Responsibilities:
- Assess residents' conditions and nursing needs, including pain assessment and management.
- Administer medications following the eight medication rights to minimize errors.
- Document changes in resident conditions, nursing care, and physician orders accurately and timely.
- Notify physicians of changes in resident conditions and follow through on orders.
- Provide thorough shift reports and participate in walking rounds.
- Implement individualized care plans based on interdisciplinary team goals.
- Supervise and delegate tasks to CNAs and supportive staff effectively.
- Participate in resident and family teaching and involve them in care planning.
- Maintain infection control standards and supervise compliance among staff.
- Remain calm and provide leadership during emergencies or stressful situations.
- Participate in orientation and performance evaluations of new personnel as needed.
- Advocate for residents' rights and treat residents and families with respect and dignity.
- Graduate of an accredited LVN program.
- Current California Licensed Vocational Nurse (LVN) license.
- Current Basic Life Support (BLS) certification.
- Ability to read and communicate effectively in English.
- Skilled nursing facility experience (Sub-Acute experience not accepted).
- Basic computer knowledge.
- Experience with Electronic Health Records (EHR).
- Additional language proficiency.
- Weekend shifts are mandatory.
- Work performed in a skilled nursing facility environment.
- Must follow facility policies and infection control procedures strictly.
- Ability to manage stressful situations calmly and confidently.
- Day shift schedule with required weekend shifts.
- Shift assignments determined by facility needs.
