Care Management Coordinator

Working remotely from anywhere in Michigan, the full-time Care Management Coordinator will perform administrative care management activities, including outreach to members, answering inbound calls, and scheduling services while serving as a point of contact for members, providers, and staff. Key responsibilities Conduct outreach to members to support care plan next steps and connect them to health plan and community resources Serve as front-line support for member and provider inquiries, explaining care plan procedures and protocols Document and maintain non-clinical member records in compliance with state and regulatory requirements Required qualifications High School diploma or GED 2 - 4 years of related experience in care management or customer service In-depth knowledge of care management support activities and community resources Experience in member onboarding and administrative support duties Ability to comply with policies and standards in a regulated environment
