We are seeking compassionate and experienced Caregivers to join our team at Touching Hearts at Home - North Houston! Responsibilities:

• Provide companion-based care to clients in their homes, including personal care, light housekeeping, meal preparation, and assistance with daily living activities

• Monitor clients' conditions and report any changes to our care management team

• Maintain accurate and detailed records of client care

• Build strong, trusted relationships with clients and their families

• Collaborate with other care team members to ensure seamless service delivery
