Outreach Specialist

Job Overview – Outreach Specialist Compensation: $26/hour Location: Staten Island, NY Schedule: Monday to Friday (In-Office, Occasional Weekends) TAG MedStaffing is hiring an Outreach Specialist in Staten Island, NY for our client, supporting community outreach, member engagement, and enrollment into care coordination services. The Outreach Specialist will conduct field-based outreach throughout the community to identify eligible individuals, reconnect with existing members, and connect individuals with appropriate services. This role requires regular community travel and direct engagement with individuals who may face barriers to accessing care. Responsibilities as the Outreach Specialist: Community Outreach: Conduct phone, in-person, and field-based outreach to connect individuals with available services and resources. Client Engagement: Engage prospective clients, complete initial screenings, and provide guidance on appropriate programs and support services. Client Support: Re-engage individuals as needed and help identify barriers to accessing or continuing services. Community Partnerships: Build relationships with community organizations, service providers, and other stakeholders to strengthen outreach and referrals. Documentation: Maintain accurate outreach records, progress notes, screening information, and related documentation within electronic systems. Community Engagement: Participate in community events and outreach activities throughout the assigned service area. Qualifications for the Outreach Specialist: Education: High school diploma or equivalent is required,with a bachelor's degree in Human Services, Social Work, Psychology, Public Health, or a related field preferred. Licensure: Valid driver's license and ability to travel throughout the assigned service area are required. Experience: 1+ year of community outreach, human services, social services, care coordination, case management, or related experience is preferred. Industry Knowledge: Familiarity with community-based services, social services, and resources for underserved populations is required. Technical Skills: Basic computer, data entry, and electronic documentation skills are required. Skills & Attributes: Strong communication, interpersonal, organization, outreach, and relationship-building skills are required, with bilingual Spanish and English skills preferred. #J-18808-Ljbffr
