Hospice RN Case Manager

The Registered Nurse Case Manager provides comprehensive hospice nursing care, developing and implementing personalized plans of care in collaboration with patients, families, and healthcare teams. Responsibilities include conducting initial eligibility evaluations, reassessing patient needs regularly, documenting care activities, and educating families on self-care techniques. The role requires delivering quality, compassionate care in diverse home environments, ensuring patient well-being across physical, psychosocial, emotional, and spiritual domains.
