Position Summary:
The Clinician is responsible for providing comprehensive counseling services to clients in accordance with organizational policies, professional ethical standards, state regulations, and accreditation standards. The Clinician conducts assessments, develops individualized treatment plans, facilitates therapeutic interventions, and maintains timely and accurate clinical documentation.
This position supports the organization’s mission by delivering evidence-based, trauma-informed, and person-centered care while promoting client safety, dignity, recovery, and continuity of care.
Requirements:Essential Duties & Responsibilities:
Clinical Assessment & Treatment Planning
- Conduct comprehensive biopsychosocial assessments, risk assessments, and clinical evaluations utilizing evidence-based and standardized assessment tools.
- Diagnose mental health disorders in accordance with DSM-5 criteria and applicable scope of practice regulations.
- Obtain and review prior treatment records, referral information, and collateral information as clinically appropriate.
- Develop individualized treatment plans with measurable goals, objectives, and interventions in collaboration with the client.
- Review and update treatment plans according to regulatory, payer, and organizational requirements.
Counseling & Therapeutic Services
- Provide individual, group, and family counseling services utilizing evidence-based therapeutic modalities.
- Facilitate psychoeducational groups and recovery-oriented programming.
- Monitor client progress and response to treatment interventions.
- Identify barriers to treatment engagement and implement appropriate interventions.
- Provide crisis intervention and de-escalation services as needed.
- Coordinate referrals for ancillary services and higher or lower levels of care when clinically indicated.
Documentation & Compliance
- Complete clinical documentation accurately, timely, and in accordance with accreditation standards state regulations, payer requirements, HIPAA regulations, and organizational policies.
- Maintain documentation including assessments, progress notes, treatment plans, discharge summaries, reassessments, referrals, and coordination of care activities.
- Ensure all entries are signed, dated, and completed within required timeframes in the electronic health record system (Kipu).
- Maintain confidentiality in accordance with HIPAA and 42 CFR Part 2 regulations.
- Participate in internal audits, quality improvement activities, and compliance initiatives.
Care Coordination & Team Collaboration
- Collaborate with multidisciplinary treatment team members to ensure continuity and quality of care.
- Communicate effectively with referral sources, external providers, case managers, probation/parole officers, family members, and community agencies as appropriate and authorized.
- Participate in case conferences, staff meetings, supervision sessions, and clinical consultations.
- Coordinate discharge and aftercare planning to support successful transitions and relapse prevention.
Safety & Professional Standards
- Adhere to organizational policies related to safety, infection prevention, emergency procedures, workplace violence prevention, and client rights.
- Maintain professional boundaries and ethical standards consistent with licensing board requirements and organizational expectations.
- Report incidents, safety concerns, abuse, neglect, or suspected violations according to organizational policy and mandated reporting laws.
- Participate in required annual trainings and competency assessments.
Supervisory Responsibilities:
This position does not require supervisory responsibilities.
Qualifications:
Education:
· Bachelor’s degree in Social Work, Counseling, Psychology, Human Services, or a related behavioral health field required.
· Master’s degree preferred.
Licensure and/or Certification:
· One of the following active licenses/certifications preferred and maintained in good standing:
o LSW
o LCSW
o LPC
o LAC
o LMFT
o CADC/CADC II or equivalent credential (if applicable)
Experience:
· Experience working in behavioral health disorder treatment preferred.
· Experience facilitating group counseling and treatment planning preferred.
· Experience with co-occurring disorders preferred.
Knowledge, Skills, Abilities:
- Knowledge of DSM-5 diagnostic criteria.
- Knowledge of trauma-informed care and evidence-based practices.
- Understanding of confidentiality regulations including HIPAA and 42 CFR Part 2.
- Strong written and verbal communication skills.
- Ability to manage crisis situations professionally and effectively.
- Proficiency with electronic health records (EHR) and Microsoft Office applications.
- Ability to work collaboratively within a multidisciplinary team environment.
