About the Role

We are seeking a dedicated Occupational Therapist (OT) for a 13-week contract opportunity in Santa Rosa, California. The ideal candidate will provide high-quality rehabilitation services in a Skilled Nursing Facility (SNF) , helping patients improve independence, restore functional abilities, and enhance their quality of life through individualized therapy programs.

Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans based on functional goals.
Provide skilled therapy interventions to improve activities of daily living (ADLs), mobility, coordination, strength, and independence.
Monitor patient progress, modify treatment plans as needed, and ensure optimal rehabilitation outcomes.
Maintain accurate documentation of evaluations, treatments, progress notes, and care plan updates.
Collaborate with physicians, nursing staff, rehabilitation teams, patients, and families to coordinate comprehensive patient care.

Requirements

Active California Occupational Therapist (OT) License required.
Minimum 1 year of Occupational Therapy experience required.
Skilled Nursing Facility (SNF) experience preferred.
Ability to work a variable schedule as assigned.
Strong patient care, communication, documentation, and rehabilitation skills.
