The Elevance Health Companies, Inc. is seeking an LTSS Service Coordinator-Clinician to assist in identifying high-risk members, coordinate their care, and work in collaboration with the nursing team. This role offers the flexibility of virtual work from anywhere in Tennessee, with in-person training sessions required. Candidates must have a valid nursing or social work license, paired with at least 2 years of experience in case management for individuals with chronic illnesses.
