CASE MANAGER

Case Manager II Location: Montgomery County, PA Position Summary The Case Manager is responsible for assessing, coordinating, and connecting individuals and families to services that support their individualized needs. This role focuses on advocacy, resource navigation, and short-term support to ensure access to behavioral health, social services, and community-based resources. Key Responsibilities Team-Based Planning Collaborate as part of a multidisciplinary team to deliver holistic, person-centered services Build and maintain relationships with community providers and partner agencies Provide referrals to behavioral health and community-based services, including RHD programs and external resources Direct Client Support Assess individual needs and determine service eligibility through office and community-based interactions Provide screenings, referrals, and resource coordination in a timely manner Assist with benefits and services including Medicaid, Social Security, housing, employment, and transportation Offer short-term, solution-focused support to promote independence and community integration Administrative Duties Complete all required documentation in compliance with RHD, funders, and regulatory standards Perform responsibilities in accordance with County Administrative Case Management (ACM) requirements Maintain accurate records and ensure confidentiality (HIPAA compliance) Attend required trainings, meetings, and maintain professional standards Qualifications Education & Experience Bachelor’s degree in Human Services or related field required

  1. year of case management experience preferred
  2. years of experience supporting individuals with behavioral health, substance use, or IDD preferred
Intermediate computer skills required Licenses & Requirements Valid driver’s license (minimum 3 years, non-provisional) required Access to a reliable personal vehicle with state minimum insurance Child abuse clearances and other state-required background checks required Core Competencies Strong communication and interpersonal skills Ability to collaborate with diverse populations and community partners Ethical practice and commitment to confidentiality Strong organizational and documentation skills Problem-solving and crisis intervention abilities Knowledge of behavioral health systems and community resources Cultural humility and person-centered approach Proficiency in basic computer systems and documentation tools Work Environment & Physical Requirements Combination of office-based and community-based work Ability to sit, stand, bend, lift (up to 20 lbs), and travel locally Regular use of computer, phone, and office equipment Compliance with OSHA, HIPAA, and all organizational policies Apis Services, Inc. and affiliate’s provide equal employment opportunities for all employees and applicants for employment in compliance with all federal and all applicable state and local laws and regulations, including nondiscrimination in hiring and employment.
