Job Summary:
A Registered Nurse (RN) specializing in Home Healthcare located in Littleton, New Hampshire. The position involves creating personalized care plans for both Home Health and Hospice patients, providing care across all stages of life in patients' homes. Candidates are required to bring their license on the first day. The role offers a flexible daytime schedule with a focus on autonomy within a multidisciplinary team.

Job Details:

• Location: Littleton, New Hampshire
• Schedule: Days, 8 hours per shift, minimum of 40 hours per week
• Required Qualifications: Valid New Hampshire Nursing License, current Basic Life Support (BLS) certification from the American Heart Association, at least 18 months of recent Home Health experience, 2-5 years of medical-surgical experience preferred, possession of a valid driver's license and auto insurance, and ability to work independently without supervision.
• Additional Information: Candidates must be proficient with electronic medical records data entry and demonstrate flexibility in handling unpredictable work situations. Experience with Hospice and supervisory references from the past year are highly preferred.
• Submission Requirements: A complete work history, Home Health skills checklist, Hospice skills checklist (highly preferred), supervisory signed reference (from the past year), current auto insurance documentation, and permanent address must be included. All licenses and certifications must be current at the time of submission.
