Job Details:

• Location: Antigo, Wisconsin
• Shift: 8 hours, varied times
• Schedule: 40 hours per week, with potential flex up to 40 hours; weekends every 4th weekend; rotation for holidays; on-call 1-2 times during the week and every other weekend. The clinician working the weekend for admissions/routines will also be on-call their assigned weekend. Response time: Clinician has 15 minutes to return answering service calls; back-up contact by clinical manager if needed.
• Certifications: Wisconsin License, BLS (Basic Life Support), valid Driver's License, OASIS certification is a plus; AHA or ARC certifications required by Aspirus.
• Experience: Minimum 2 years in home health, including travel experience, with proficiency in electronic medical records (EMR) such as Epic, Oasis documentation, wound care, IV, ports, lab draws, chronic disease management.
• Education: Associate Degree or Certificate in Nursing.
• Additional Qualifications: Hardworking, excellent time management, resilient, adaptable to change, dependable, compassionate, and positive attitude.
• Travel Requirements: Approximately 50 miles daily, supporting 20-25 patients with an average of 4.5 visits per day.
• Float: Potential to float to other territories if needed during low census periods.
• Vaccinations: Flu and COVID vaccines required; mask-wearing policies during flu season if declined vaccination.
• Clinical Responsibilities: Admitting patients via Oasis, episode management, care coordination, and timely documentation in EMR.
• Start Date: 06/22/2026.
• Minimum Experience: 0 years listed, but a minimum of 2 years in home health is required.
• Special Notes: Candidates must have reliable transportation and be prepared for rotation schedules including holidays and on-call duties.
