As a Home Health Physical Therapist with Elara Caring, you'll deliver personalized care to patients throughout the MaldenMelrose territory. You'll have the independence to manage your day, the time to focus on one patient at a time and the support of an experienced interdisciplinary team.

What You'll Do:

• Evaluate patients' mobility, strength, balance and functional abilities
• Develop individualized treatment plans focused on meaningful, achievable goals
• Provide hands-on therapy that promotes safety and independence
• Help patients recover following illness, injury, surgery or hospitalization
• Educate patients and caregivers on exercises, equipment and fall prevention
• Monitor patient progress and adjust treatment plans as needs change
• Collaborate with physicians, nurses, therapists and clinical leaders
• Complete accurate and timely clinical documentation
• Provide guidance and clinical support to therapy team members when appropriate

What You'll Bring:

• Degree from an American Physical Therapy Association-approved program
• Current, unrestricted Massachusetts Physical Therapist license
• Strong clinical judgment, communication, and organizational skills
• Ability to work independently and manage a field-based schedule
• Willingness to travel throughout Malden, Melrose, and nearby communities
• Valid driver's license, dependable transportation, and current auto insurance
• Sit, stand, bend, lift and move intermittently and be able to lift 50100 lbs.
