This position is structured as a remote role, offering flexibility while still fostering a tight-knit team environment. Our work hours are Monday to Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs. The role demands a flexible scheduling approach to accommodate home visits, assessments, and urgent care situations. We utilize a variety of digital communication tools and electronic health record systems to support our clients effectively.

Key Responsibilities\n

Your responsibilities will include:

• Conducting comprehensive psychosocial assessments of senior clients to identify emotional, mental, and social factors impacting their well-being.

• Developing and implementing individualized care plans in collaboration with clients and their families, encompassing specific goals and therapeutic interventions.

• Providing one-on-one counseling and emotional support to help clients navigate life transitions, family dynamics, and losses.

• Advocating for clients’ rights, ensuring access to essential community resources, and guiding families to appropriate services.

• Collaborating with an interdisciplinary team, including healthcare providers and caregivers, to coordinate holistic care delivery.

• Regularly visiting clients at home to assess living conditions and monitor their mental and emotional health.

Qualifications\n

Ideal candidates should possess the following qualifications:
