About the Facility Peachtree Centre is a 151-bed skilled nursing facility with 40 short-term rehab beds. Offers comprehensive short- and long-term rehabilitation and outpatient services Features a state-of-the-art rehab gym with community reintegration to help residents achieve their highest level of function Join a supportive in-house rehab team under outstanding leadership About the Location – Gaffney, SC Located just 40 minutes from Charlotte, NC, and 30 minutes from Spartanburg, SC. Responsibilities Review resident information and evaluate its impact on assessment and treatment. Conduct and document thorough functional assessments using standardized procedures. Develop and implement individualized treatment plans with achievable goals. Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records. Instruct and supervise therapy assistants as needed. Enter treatment and progress notes for each session. Revise treatment plans as needed throughout the course of care. Document treatment in clear, concise discharge summaries. Orient residents to services and treatment procedures appropriate to their needs. Collaborate with nursing staff, residents, and families to maximize functional abilities. Educate residents and caregivers on the continuation of home exercise and therapy. Assist with referrals for additional services as needed and maintain knowledge of community resources. Participate in rehabilitation team conferences and other facility meetings. Access continuing education opportunities to maintain clinical competency. Participate in quality improvement initiatives. Follow regulatory, professional, and facility standards. Perform other duties as assigned to provide quality care. Understand payer reimbursement and allocate therapy resources efficiently. Support Ahava’s mission and project a professional image.
