CBHH Care Coordinator - South Portland Middle School

Position Overview Children’s Care Coordinator – Children’s Behavioral Health Home (CBHH) – South Portland Middle School. This role provides care coordination to students in the community school setting as part of a team-based case management model that includes a peer family support specialist, nurse care manager, and medical/psychiatric consultants. Key Responsibilities Meet with families to assess needs and develop care plans. Support families at school meetings and help them navigate benefits and services. Maintain timely and accurate documentation. Collaborate with interdisciplinary team members to coordinate holistic care. Participate in individual and group supervision with no on‑call, night, or weekend duties. Qualifications A bachelor’s degree in psychology, behavioral health, social work, or a related field; or a bachelor’s degree in another field with at least one year of full‑time equivalent human services experience. Ability to work independently and collaboratively. Flexibility to work evenings; must be able to meet physical requirements such as climbing stairs and carrying small children or materials up to 25 pounds. Proficient in Microsoft Office and email. Must pass a criminal background check, child protective service check, and sex offender check; not on state or federal suspension and disbarment lists. A valid driving record if driving is required for meetings, trainings, or client visits. Schedule & Location Full‑time, 40 hours per week; Monday through Friday with possible evening hours. Work will primarily be performed in the school and community; occasional remote paperwork is allowed. Benefits Paid time off with 9 holidays per calendar year and up to 3 floating holidays.
