Account Manager (Oakland/Alameda County)

Health Link Home Health and Hospice has been committed to providing exceptional healthcare services to patients in the San Francisco Bay Area in their homes. We are looking for a skilled Customer Success Representative to advocate for our home health and hospice services. At Health Link Home Health and Hospice, your primary responsibility will be to leverage our community relationships to build and reinforce trust in our programs. Your role is to assist the front line of our home health team and give more patients the opportunity to be cared for by our clinicians. You will work with the Marketing Team and Director of Community Relations to execute a sales and marketing plan and achieve our goals. A keen grasp of sales and marketing and understanding of the home health and hospice industry is a plus. Regardless of experience in home health, you must digest information rapidly and learn quickly. Responsibilities Assist in implementing a comprehensive marketing and sales plan. Develop and maintain relationships with community partners including hospitals, doctors' offices, clinics, senior living communities, and skilled nursing facilities. Ride alongs with current team members, respecting existing relationships, and developing your own. Make calls to schedule meetings and visit in person to present our services, including extensive follow‑up. Collaborate with the intake team to coordinate referrals and serve as liaison between community partners and internal staff; many messaging and coordination calls. Represent Health Link at relevant community events and meetings to build awareness, develop relationships, and expand our network. Manage your own budget, track expenses, and ensure effective use of resources. Identify, target, and develop new referral partnership opportunities. Always have your phone and respond promptly unless on a planned vacation. Maintain strict calendar discipline. Maintain strict CRM logging discipline.
