Nurse Practitioner or Physician Assistant

About the Role Practice medicine in a way that prioritizes time, connection, and meaningful patient relationships. As a Nurse Practitioner or Physician Assistant in Physician Services, you will provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings. This role offers flexibility, collaboration, and a supportive clinical environment that removes unnecessary administrative burden. You will benefit from no weekends, holidays, or on‑call requirements; a flexible schedule; a limited patient census that allows you to spend more meaningful time with each individual; strong back‑office support, streamlined credentialing, structured onboarding, and ongoing collaboration with physicians and peers to focus on clinical decision‑making and patient care from day one. In this role, you will conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team members. Working within a collaborative practice framework, you will serve as a key clinical decision‑maker while helping patients and families better understand diagnoses, manage chronic illness, and avoid unnecessary hospitalizations. Location Office Location: Jackson, MI Main Service Area: Oakland County and surrounding areas This role is a high‑travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes. Hours Monday to Friday, 8 a.m. to 5 p.m. Required Qualification Master’s or doctoral degree as a Board‑Certified Nurse Practitioner or Physician Assistant Current State License as a Nurse Practitioner or Physician Assistant Reliable transportation, valid driver’s license, and valid automobile insurance coverage Preferred Qualifications Minimum 2 years clinical experience in a post‑acute setting as a Nurse Practitioner or Physician Assistant Adult, Family, or Gerontology certification Experience with geriatric patients Essential Skills and Competencies Nursing skills as defined by accepted nursing standards Ability to assess and respond to the needs of patients, families and caregivers in post‑acute care settings Skill in nursing practice and ability to cope with family emotional stress and tolerance of individual lifestyles Ability to handle multiple priorities, documentation requirements, and deadlines Pay Range: $110,000—$120,000 USD How We Care for You Minimum of 3 Weeks Paid Time Off (PTO) Medical, Dental, and Vision Insurance HSA and FSA options including Dependent Care Company‑paid Short Term Disability Company‑paid Life Insurance 401(k) with Employer Match Mileage Reimbursement Company Vehicle Program for field roles Pet Insurance ID and Fraud Protection Background Screening Employment is contingent upon the successful completion of a background check performed by a third‑party administrator, the Michigan Long‑Term Care Partnership, in compliance with the Fair Credit Report Act.
