Minimum Requirements Bachelor's, Master's or Doctoral Degree in Physical Therapy from accredited schools approved by the APTA Active State License is Required to Start the Assignment BLS Certification May Be Required from AHA or ARC Physical Therapy License Master's Home Health experience Experience creating OASIS and evaluations Assignment Details Monday through Friday. 8:00 AM to 4:30 PM. No on-call or weekend requirement. Productivity expectation: 30 visits per week. Mileage reimbursed between visits, from first patient to last patient. Coverage area includes Lamar, Fannin, and Grayson counties. Dress code: Grey, Black, Navy or Burgundy.
