Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment that promotes healing and wellness. Nursing practices are based on evidence, current research, and policies and procedures. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and other disciplines. Responsibilities include coordinating, ensuring, and providing direct patient care activities on the unit. This includes nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. Patient and family-centered education is provided as well as care coordination and individualized treatment planning. The RN performs and documents comprehensive assessments in accordance with scope of practice and policies. They consider the patient's culture, age, and developmental level. The RN identifies patient risk factors and presenting complaints. They obtain health information from family or others as necessary. Objective and subjective data is collected, and initial admission assessments are verified. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Actions and interventions are designed according to the patient's culture and age. Desired outcomes are determined in coordination with the patient and family in accordance with the medical plan of care. Patient care therapies and procedures are fully implemented within the scope of practice and established guidelines. The RN explains procedures, treatments, and processes to the patient and family. Initiatives are taken to alleviate patient stress and discomfort. Changes in patient condition are recognized, responded to, and communicated appropriately. Ongoing assessments and responses to procedures and therapies are conducted and documented. Tasks are delegated appropriately, and oversight is provided to team members. The RN monitors accuracy and completeness of delegated patient care provided by other team members. Educational needs of patients are identified and addressed per prioritized problems. The discharge process is facilitated by executing discharge orders and assuring performance of discharge teaching. Feedback is provided in patient/nurse assignments considering available staffing, unit activity, and patient acuity. Active participation in unit-based activities in support of organizational values is encouraged. A positive group dynamic and healing environment through words and actions is promoted. Nursing excellence is assured in accordance with accepted standards and evidence-based practice. Participation in quality improvement activities and core measures is actively encouraged. A holistic perspective regarding patient care is utilized.
