RN Case Manager | Hospice & Home Care

Take charge of developing, implementing, and managing personalized patient care plans in a hospice and home care setting. Collaborate closely with physicians and interdisciplinary teams to optimize patient outcomes. This full-time day role offers a $5,000 sign-on bonus.

Essential Functions:

• Create individualized care plans based on medical and nursing diagnoses for patients ranging from newborns to geriatrics.
• Deliver skilled nursing services aligned with the care plan.
• Complete accurate OASIS assessments as required.
• Work with physicians, patients, families, and other care providers to adjust care plans as needed.
• Supervise Home Health Aide services according to policy.
• Participate in on-call rotations to ensure 24-hour nursing availability for hospice, home care, and private duty patients, including scheduled visits and admissions.
• Maintain timely and precise documentation following agency guidelines.
• Foster positive relationships with patients, families, colleagues, physicians, and referral sources.
• Encourage patient independence by setting care goals and involving patients and families in discussions about medical issues, testing, medications, and self-care.
• Monitor patients' physical, mental, emotional, and spiritual well-being and collaborate with team members accordingly.
• Adhere strictly to HIPAA regulations regarding Protected Health Information (PHI).

Minimum Knowledge and Skills:

• Proficiency with MS Office and keyboarding skills.
• Strong communication abilities.
• Effective multitasking and prioritization.
• Basic nursing principles application.

Preferred Knowledge and Skills:

• Clinical nursing skills and familiarity with Home Health OASIS Data Set.

Behavioral Skills:

• Ability to manage conflict, inspire others, and lead process improvements.

Education and Experience:

• Graduate of an accredited nursing program with several years of clinical experience.

Preferred Education and Experience:

• Bachelor of Science in Nursing (BSN), prior hospice or home health experience, and certification as a Hospice and Palliative Care Nurse.

Current Registered Nurse license in the state of Kansas, Basic Life Support (BLS) certification within three months of hire, valid Kansas driver's license, and auto insurance meeting state requirements.
