We are currently hiring a Family Caregiver Support Specialist. In this position you will Respond to requests for information from prospective consumers and community agencies regarding the Family Caregiver Support Program and Medicare’s GUIDE Program. Assist consumers within the parameters of the MFCSP and Medicare’s GUIDE Program as a Care Navigator. This includes telephone calls, in-home visits and telephone assessments, researching appropriate resources and caregiver topics, assistance with caregiver support groups and workshops, managing computer documentation and participating in community outreach.

The pay range for this position is $ 32.34 - 33.80 per hour for34 hours per week plus unpaid one hour lunch each day.

• Ability to communicate and work with the elderly and community resources serving the elderly population.
• Excellent computer skills required.
• Ability to work independently and professionally.
• Valid MA driver’s license and car required
