Job Description Job Description Schedule Monday 0700-1500 Tuesday 1100-1900 Friday 0700-1500 Every other Saturday and Sunday 1100AM-1900 The Hospice CNA provides compassionate, high quality personal care to hospice patient under the direction and supervision of a RN and in accordance with the patient's individualized pland of care. The CNA promotes comfort, dignity, and quality of life while supporting patients and their families throughout the end of life journey. Provider personal care Assist patient with mobility, transfers, positioning, ambulation, and range of motion exercises as directed. Ability to safely use a Hoyer lift. Provide emotional support and compassionate care to patients while respecting cultural, spiritual and personal preferences. Maintain accurate and timely documentation in EMR. Maintain patient confidentially Maintain supplies. Attend staff meetings. Perform other duties as assigned.
