Heart Failure Nurse Practitioner: Cardiac Care & Impact

Montefiore Hudson Valley Collaborative

Montefiore Medical Center in New York seeks an experienced Nurse Practitioner to provide comprehensive care for cardiomyopathy and heart failure. You will lead patient care coordination, participate in quality initiatives, and mentor staff within a multidisciplinary team.

You will assess, diagnose, treat, and coordinate care across locations, with travel between Medical Arts Pavilion and Hutchinson Metro Center as needed.
