Family Practice - Geriatrics Physician

ThedaCare, Wisconsin's largest non-profit healthcare system, is seeking a Senior Care Physician to join our established team in the Clintonville and New London region. You will be responsible for working with PALTC patients in community living facilities, developing relationships while administering high-quality patient care.

Position Details:

Full-time (4-10s) or Part-time options available with flexible schedules!
Location: Clintonville and New London
The team consists of 6 Physicians and 10 APCs
2-3 SNFs a day (patient schedule built within EPIC)
On-Call by phone only: 1:11
Patient census: 8-12
Support staff includes RNs and schedulers who help to coordinate provider visits, communicate with facilities, and triage phone calls.
27 Facilities total between skilled nursing and assisted living facilities
Large opportunities for growth, such as Medical Director roles in the nursing homes that we serve
